Every few months a new claim surfaces in online hair communities about whether hair extensions are "safe" — usually triggered by a viral video, a dermatologist's offhand comment, or a brand's marketing language designed to differentiate by implying competitors are high-risk. Our assessment: the clinical evidence is narrower and more specific than either the fear-mongering or the reassurance suggests.

What Traction Alopecia Actually Is (And When Extensions Cause It)

Traction alopecia is hair loss caused by sustained tension on the hair follicle. It is real, it is documented in clinical literature, and it is caused by extension methods when they are installed incorrectly, worn too long without maintenance, or applied to a client whose natural hair density cannot support the weight and tension of the chosen method.

The dermatology position on this is neither alarmist nor dismissive. A 2019 review in the Journal of the American Academy of Dermatology documented traction alopecia as more common in populations with a cultural practice of tight hairstyling — braids, weaves, and certain extension installation patterns — and explicitly noted that the condition is reversible in its early stages but can produce permanent follicular scarring if tension persists over years.

The clinical takeaway for extension clients is specific: the problem is tension, not hair extensions as a category. A correctly weighted, correctly tensioned, professionally installed set does not produce the follicular tension that causes traction alopecia. A set that is too heavy for the client's natural hair density, installed too tightly at the weft connections, or maintained past its service life does. Scalp Health: The Evidence Is Genuinely Mixed

Contact dermatitis from extension adhesives is documented. Keratin bond adhesives contain ketone solvents and adhesive polymers that produce allergic contact dermatitis in a subset of clients — patch testing before first installation is the clinically recommended practice, and most dermatologists who see extension-related skin reactions report that pre-installation patch testing was not performed. The reaction rate is low in the overall population but concentrated among clients with existing adhesive or latex sensitivities.

Scalp microbiome disruption is a newer research area with less settled evidence. Extensions that reduce airflow at the scalp alter the moisture and pH environment where the scalp microbiome lives. A 2022 pilot study (small sample, not peer-reviewed at publication) suggested measurable microbiome composition shifts in participants wearing sewn-in extensions for 8+ weeks. Similarly, influencer content claiming extensions "cause hair loss" without specifying the method, installation quality, and client history conflates installation malpractice with product category risk. Hair loss following extensions is real; the question is whether it follows from the method or from a specific, addressable failure in installation or maintenance.

Our Assessment

The dermatology evidence supports extensions as a low-risk category for most clients when professionally installed, appropriately weighted for the client's natural hair density, and properly maintained on a professional service schedule. The risk profile increases with: heavy methods on fine/thinning hair, extended wear without maintenance, adhesive sensitivity in clients without prior patch testing, and installers without method-specific training.

The "are extensions safe?" question, asked without those qualifiers, doesn't have a useful answer. The qualified version — safe for whom, installed by whom, maintained how — has a much clearer one. The dermatology position is neither the blanket endorsement extension brands prefer nor the categorical alarm some viral content implies. It is specific, conditional, and consistent with the evidence base as it currently exists.
